The Form Assembly - Form Builder

New: Open this form in the new version of the Form Builder

Quick Start

To give a name to your new web form, click on the 'unnamed form' link on the left.
To add a new form field, click on the 'Add a Question' button.
You can organize your form with sections. Click on the 'Add a Section' button. Your next question will be created inside the new section.
To rearrange your questions, drag & drop them in the view on the left.
When your form is ready, click the 'Save' button. You will have the option to login or register if you have not done so already.
