The Form Assembly - Web Form Builder

Welcome to the Web Form Builder (v2.4)

To give a name to your new web form, click on the 'unnamed form' link on the left.
To add a new form field, click on the 'Add a Question' button.
Sections can be used to organize your form and to change how enclosed questions are layed out.
To rearrange your questions, drag & drop them in the form outline on the left.
When your form is ready, click the 'Save' button. You will have the option to login or register if you have not done so already.
