Intake form
Keshet’s newest project involves a revamped system that they’ve implemented to better track the intake process when staff members first make new connections. To have an aggregate view of what was happening during the intake process, they worked diligently to set up custom objects in Salesforce that would receive all the information collected on a form.
A key strategy was to make the FormAssembly intake form accessible from within Salesforce via a custom link on the Account and Contact page layouts.
When a Keshet staff member clicks the “Create an Intake!” link, FormAssembly’s Salesforce Prefill Connector populates the form with Account and Contact information already in Salesforce. They can then verify this information during the intake conversation, and update it if needed. This makes the process faster for staff and ensures the most current data is on file.
Intake forms now include conditional logic, designed especially for the types of intakes that Keshet regularly completes (consultations, training, and speaking engagements).
Nemser-Abrahams worked closely with staff who completed intakes to create a system that was designed with their everyday work in mind. To make the forms user-friendly, they implemented descriptive text and instructions for clarity. They also added custom CSS and repeatable sections to make forms more visual and convenient for users who need to submit them quickly while on the phone or working through a fast-paced consultation.
“Having the prefill connector is also a handy feature because it means the staff member can focus on the actual intake,” Nemser-Abrahams said.

Keshet’s donation forms are used in conjunction with the Authorize.net connector, which enables them to accept and process credit card payments through their forms.

Keshet uses FormAssembly to create registration forms for all of their events. For programs with complex registration processes, they can use FormAssembly’s features to establish workflows to keep information organized. The organization currently makes use of over 50 active forms.
